This form is to be completed by any individual, entity, or collective who currently exists but lacks the requisite documentation to prove that they have previously existed.
Please note: submitting this form does not constitute proof of existence. Existence must be independently verified by a licensed Ontological Auditor (Grade 3 or above).

- Memories (note: applicant's own memories are not accepted as evidence — see Kraal v. The Concept of Self, 3.7 billion GCY)

I, the undersigned, declare that I currently exist and wish to have also existed previously. I understand that filing a false Retroactive Existence Application is punishable by retrospective non-existence, which I acknowledge would make this form paradoxical.
